The Resource Coordinator will provide quality transitional services to patients to enhance the on-going recovery process.
Documentation Responsibilities: Maintain required records, reports, and statutes for administrative purposes.
Complete reports in a timely manner and with professional content.
Maintain, update and review patient charts for accuracy and completeness.
Compile and distribute calling/visiting lists for patients, based on the direction of the placing agency and the program’s policies.
Communication Responsibilities: Coordinate with all service areas (Psychiatric, Psychological, Nursing, Family) regarding recommendation of transitional services
Ensure that all documentation to referring agency is delivered in a timely fashion. These documents include, but are not limited to: weekly reviews, Staffing Reports, discharge reports,
Participate in intake meetings at the time of admission of assigned patients and cross train the position of Admissions Specialist
Update electronic and non-electronic records with patient information and discharge data as it occurs.
Participate in the process for continuing care: make follow up phone calls to patient, referent, collateral contacts as appropriate. Document all follow-up and present progress in aggregate form
Other duties as assigned.
CPR and first aid certified.
Must know and understand all rules, regulations, and requirements of the assigned program.
Minimum of two years continuous freedom from chemical use problems.
Must be 18 years of age
Strong verbal skills and ability to communicate with a variety of people from diverse cultures, socioeconomic and educational backgrounds.
Flexibility to meet a variety of changing priorities
Computer skills to draft emails, memos, and reporting
