The Continuing Care Coordinator is responsible for supporting/mentoring clients and may attend groups, provide individual peer recovery services, participate in phone and/or individual recovery coaching, and continuing care support to clients for when the client leaves the program. In carrying out the position’s requirements, confidential working relationships with leadership, clinical staff, administrative staff, and clients must be maintained. Must follow all state and federal guidelines, exercise sound judgment and discretion, demonstrate initiative, trustworthiness, cooperation, and dependability while performing assigned tasks and responsibilities. 

Pay: $23 - $27 / hour  
Shift: Part-Time, 1st Shift

Responsibilities

This job is expected to complete the following functions. This is not an all-inclusive list. This job may be assigned other functions or tasks as business needs dictate. This is meant to be a general description of the most essential duties only. 

Clinical Procedures
  • Ensure ROI’s are in place for guests continuing care providers. 
  • Make 3rd party calls, when ROI’s are in place to notify of continuing care appointments and coordinate treatment services. 
  • Document meetings with guests, 3rd party calls, and interactions pertaining to guest and/or continuing care coordination. 
  • Communicate on a regular and timely basis with other members Executive Director. 
  • Advocate for the best interests of individual clients. 
  • Attend mandatory facility in-services and staff development activities as scheduled. 
  • Assist in orientation of new employees and interns, if assigned 
  • Provide legible, written documentation of client notes/ progress in the EHR system and must be completed by program and DHS regulations. 
  •  Work closely with both the clinical and medical teams to ensure guests are set up with continuing care services based on medical and clinical recommendations.

Skills & Qualifications

Required:
  • Must be at least 21 years of age and must be free of substance use problems for at least one (1) year immediately preceding their hiring and must sign a statement attesting to that fact.
  • Must possess a minimum of high school diploma or General Educational Development (GED).
  • Must have completed a curriculum designated by the commissioner that teaches specific skills and training in the domains of ethics and boundaries, advocacy, mentoring and education, and recovery and wellness support
  • Health Insurance Portability and Accountability Act (HIPAA)
  • Business and office practice/procedures
  • Healthcare industry, especially substance abuse and mental health services
  • Technology proficiency with various software packages including EHR/EMR and keyboarding skills
  • Continuous quality improvement
  • Very detailed and organized
  • Related with public, peers, subordinates (if applicable), and leadership staff
  • Critical thinking and problem solving
  • Strong customer service
Knowledge of:

Preferred:
  • Preferred Experience: Previous work related experience in a mental or behavioral healthcare settings
  • Preferred Education: Bachelor’s degree in related field
Physical Requirement 
  • Hearing        Ability to hear staff, patients, incoming calls, and callers
  • Vision           Ability to see computer screen, paper documents, patient charts
  • Hearing        Will experience moderate noise levels
  • Speaking      Ability to speak to staff, patients and external customers/vendors
