Conduct brief biopsychosocial assessments to determine program fit and level of care
Build rapport with clients, families, and referral sources to support engagement
Coordinate admissions, including verifying funding, arranging transportation, and addressing barriers to care
Initiate and complete documentation (e.g., progress notes, treatment plans, group notes) within required timeframes
Monitor and manage outpatient census and waitlists
Support clients in navigating external systems (housing, financial aid, transportation)
Collaborate with multidisciplinary teams to develop and implement individualized treatment goals
Provide direct case management support to patients as needed
Maintain accurate records and ensure warm hand-offs to appropriate care teams
Valid license (LSW, LGSW, LISW, LICSW, LADC) or 4+ years of case management experience
Experience with co-occurring disorders and chemical dependency
CPR certification (or ability to obtain prior to hire)
Valid MN driver’s license
Must be free of substance use problems for at least one (1) year immediately preceding their hiring and must sign a statement attesting to that fact.
Strong verbal and written communication skills
Ability to communicate with diverse populations
Knowledge of the 12 core functions and systems theory
Organizational skills and attention to detail
Flexibility to manage changing priorities in a fast-paced environment
Proficiency in documentation and communication tools
Ability to work independently and collaboratively
Experience in outpatient or residential substance use treatment
Background in sales, marketing, or business development
Hearing Ability to hear staff, patients, incoming calls, and callers.
Vision Ability to see computer screen, paper documents, patient charts
Hearing Will experience moderate noise levels
Speaking Ability to speak to staff and external customers/vendors
Sitting Up to 6 hours per day
Stand/Walking Up to 2 hours per day
Lifting Up to 15 Ibs
Travel Up to 15% by car
